Accreditation Visit (AV) and Thematic Pathway for Reaffirmation (TPR) Confidential Team Recommendation Form

This form is used by Accreditation Visit and Thematic Pathway for Reaffirmation Visit teams to convey its recommendation to the Commission.

To become accredited by WSCUC, Institutions must submit the application for accreditation form along with the candidacy/initial accreditation fee.

Change in Ownership Implementation Form

Institutions must submit this form regarding Changes of Ownership, Mission or Form of Control within 30 days after implementation of the change.

Commission Member Qualifications and Nomination Form

The nominator completes this form and submits it with the curriculum vitae of the commission member nominee during the annual nomination period.

This form is used to submit a complaint to WSCUC and includes the submission instructions and links to relevant policy documents.

Credit Hour and Program Length Review Form

One of four forms that WSCUC uses to address instititutional compliance with federal regulations, this form is completed by teams in evaluating the reliability and accuracy of the institution’s assignment of credit hours and requirements related to program length. During the Accreditation Visit, teams complete this form and add it as an appendix to the Team Report.

Distance Education Review-Team Report Appendix Form

A completed copy of this form should be appended to the team report for all comprehensive visits to institutions that offer distance education programs and for other visits as applicable.

Expense Reimbursement Form

Representatives of WSCUC submit this form for reimbursement out-of-pocket expenses incurred while attending meetings of the Commission, when serving on accreditation review teams, or performing other assigned duties.

Federal Compliance Forms

There are four forms that WSCUC uses to address institutional compliance with some of the federal regulation’s affecting institutions and accrediting agencies. During the visit, teams complete these four forms and add them as an appendix to the Team Report.

Insider Trading Agreement and Disclosure Form

This form is signed by WSCUC staff, review team members, committee members, and commissioners in agreeing to abide by the Ownership of Stock in For-Profit WSCUC Institutions Policy (Insider Trading Policy)
